What is going on in this ultrasound?

Very early on in pregnancy and this was an OB appt to confirm things are developing. I was taking a lot in all at once, so I didn’t REALLY look at the print out until today. I’m not sure, but does anyone have an idea or take on what is going on to the right of the gestational/yolk sac? Tried to look it up and now I’m just curious.
