How to convince my doctor that i need a specialist

So about 4 months ago I went to the doctor due to period cramps that would make me faint and get fevers. I also started having near constant abdomen pain which painkillers didn’t help with. I started birth control pills, which gave me 2 week period cramps. And a month or so ago I got an iud (mirena) which gave me the worst pain I had ever experienced. I live in a country where going to the doctor is last resort and we had to call the er because of the pain. After the spiral I also couldn’t eat anything, move properly, and sometimes I couldn’t get out of bed because it hurt so bad. I’m having it taken out before my vacation, but I seriously need advice on how to convince my doctor or gynaecologist that I NEED someone who will help me properly.
