Why are my only options an IUD or hysterectomy and no diagnosis?

Hi, so I have had long, painful, intense periods my entire life. The past few years I have gotten them mostly suppressed with Slynd/drospirenone. The past year I have gotten intense, painful cramping and bleeding even though I havent missed any days or changed my BC. Im not even going to call it breakthrough bleeding because it straight up isnt it. Breakthrough bleeding shouldnt be painful or come on and off for weeks at a time over the course of months. I had gotten an ultrasound order from my PCP and it found likely adenomyosis, despite me being on BC. When I followed up with an obgyn she asked if I wanted an IUD or a hysterectomy. I asked if we could maybe figure out whats going on, and she said it doesnt really matter because the treatments are the same. I have a lot of other health issues including an undiagnosed auto immune disease, so an IUD would be the same as what Im currently doing, and I dont know if a hysterectomy would actually fix the issue because we dont know what the issue is. Is this just how all gynecology works? Does it matter who I see if every one keeps asking if I want them to shove stuff in or scoop stuff out? I feel so weird taking out a whole organ without knowing what the problem is. You wouldnt do it to your kidney or liver or lung. I understand their idea of the solution is to make me feel better but how does that work without knowing what the problem is?
